If you launch a surprise nuclear strike at another nuclear power, you are trying to destroy as many of their nukes as possible in the very first moments of the attack. In the case of Russia vs China, Russia would have to try and take out all or almost all of China's SRBMs, MRBMs, IRBMs, ICBMs, SLBMs, GLCMs, ALCMs, and strategic bombers, because any and all of these are useful against Russia due to China being right next door to it. I'm not sure what you mean by "within one class of weapons", but if Russia tries to first-strike China, then all of these classes of weapons have to be taken out in one go.That assumes you launch everything within one class of weapons at the first country. It's doubtful that would happen.

Uhh, no. "Launch it all" has not "given way" to "precision strike". Those two terms don't have anything to do with each other. "Launch it all" refers to the concept of "use it or lose it" in which if you don't use your nukes your enemy may destroy them. "Precision strike" refers to nukes getting smaller allowing more of them to fit on a missile carrier because they are more accurate and don't need a large yield to compensate for a large CEP. Again, these have nothing to do with each other. More accurate warheads don't change the calculus of the first-launching nation needing to destroy as much of the victim nation's nuclear arsenal as possible so that its retaliatory capacity is reduced or eliminated.Maybe... This really depends on what doctrine or strategy a particular country follows when conducting a nuclear war. The above scenario (launch it all) was rather typical of the 50's and 60's. But as accuracy and destructive power of a single warhead has increased (as well as the variety of delivery mechanisms), it has given way to a strategy of a precision strike. But, again that is all about guessing what your opponent will do, as well as knowing where everything your opponent has, is located.
