The most recent use of torpedo attack (in battle) against surface ship that I know of is the sinking of the ARA Light Cruiser General Belgrano (~12,000 tons) by HMS submarine Conqueror on May 2, 1982. It was the first use of a nuclear-powered submarine in war.
The HMS Conqueror used 3 conventional "straight running" Mk 8 mod 4 torpedos, each with 800 lb / 363 kg warhead, of which 2 hit the General Belgrano and sunked it:

The MK 8 torpedo that the British submarine used was originally designed in 1911 by Bliss-Leavitt, as USN's first 21" (533mm) torpedo. :rofl:
