I think part of the issue is that NATO equipment was designed under the assumption they would always operate under the cover of air power, or at the very least, contested air space but with enough leeway for operations. Ukraine doesn't have that so the results are evident.
It wasn't designed with this assumption, you can't really design it in. Moreover, say, Bradley was designed to operate under integral SAM cover - but systems failed to materialize due to the end of CW.
