Proper procedure would be to cycle the bolt and load a new round, which is how most of these weird jams should be resolved.
Using the forward assist to potentially jam in a defect round, could've gotten him killed.
I'm with Stoner on this one, the forward assist is not necessary and its only use is to give grunts something to push in stressfull situations.
The only situation where it's useful is when you want to load a round withouth making noise, which is an extremely niche use.
It happened with the original AK12 and I think could in the Galil Ace. Because of the Bolt handle being part of the bolt and riding in the ejection port. If your hand is in position to obstruct the bolt handle it’s going to obstruct the ejection. On the original AK12 and Ace they have left side bolt handles.A counter point , just about all of the Ak74 , Ak 47 , family has the reciprocating charging handle .

Now I remember why the first three pictures look similar. The gal who made the QBZ 191 mod for F4 had this as an attachment all the way back in January of this year. I would think it more about training rather then the gun ,people tend to put their none shooting hand in a way that they get hit by the charging handle of the SCAR . And at that point it easier to change the gun then the user , since the for then main user base of the SCAR ,SCAR isn't the first rifle they got trained on.

The QBZ191 has no issue with this, I mean there's a lot of space in front of the bolt to hold the gun more than even the ak74m unless that gun is modded with 3rd party hand guard that extends further. With the stock ironsights unlike in the old akm and 74m, you can even c-clamp and it works normally as we've seen in videos posted on this very forum (with the akm, ak74m you block the ironsights). The gun has a bolt release, the magazine catch is extended so you can even use your shooting hand to release the magazine sort of like an AR with the button press but this is with a rock and lock magazine (We've seen this feature in videos posted here too). Aside from rock and locking magazines there doesn't seem to be any disadvantage if we are to talk of pure speed. The key thing is that the gun is compatible with existing magazines (thus having such a magazine catch), ammunition, and can be attached with commercial products and other gadgets, since afterall the picatinny rails is what most devices have been stadardized upon at this rate.
Apparently newer SCARs have a separate cut out for the charging handle which doesn't reciprocate but at this point are you making a service rifle for soldiers to be mass produced or are you overengineering? What else really needs to be added to this gun at this point aside from addressing any quality control issues and minor stuff? the capability for off the shelf compatibility with attachments is already there, the safety switch, bolt release, etc is all what you see in a lot of modern service rifles. In terms of using the gun compared to something like ARs, the big difference is the magazines (I presume the chinese government took compatibility with current gear into account), and the charging handle, with the charging handle you'd spend time reaching for it with one hand or another on an AR type weapon just as you'd do with a QBZ 191, thus the only "downside" speedwise would be rock and locking the magazine (especially now that it's shown in videos you can release the current magazine with the shooting hand) but we have to take into acount of the chinese military and what equipment they have. Plus the manual of arms is very similar to an AK which I'm sure many chinese are familiar with throughout the militia, officers, police, veterans, some soldiers, etc.
