It may not be coming from the doctor but just a miss translation by the news report.

As to how did that happen, I'm guessing somehow the toothpick was placed near her body when someone or something bumped into it.

For some reason some chinese people I know translate the chinese word for mute to "dumb". I think its a language thing.
