Well for the record the world dont support killing civilians, but
1 - Everyone in Israel is IDF by law
2 - Iran is simply respecting Israel's values even if they dont agree with it. Its the Golden Rule as they say
Now we see why you should show some baseline morality in war. If Israel wasn't bombing hospitals in the past, hitting a hospital will make it really politically ugly for Iran, and to avoid that, Iran may refrain from targeting something adjacent to a hospital. Now Iran can just do that, and no matter what they hit, people can just say that's their target and even hospital being targeted can be justified. Such freedom in operation is handed over to Iran by Israel itself.
