I would depend on the deployment system. Tear gas can be employed in a canister or a shell.
If a Shell then it could have been a negligent discharge. the trigger is engaged and a shell is fired in the hull.
If A canister which is like a grenade, then if the pin came loose and was not caught in time then the fuse would have gone off.
If you look at the picture of the Soldier heaving behind him is a M4 and around that are CS gas grenades, and grenade shells. any of which could have done the job also not the soldier has a the launcher for those shells.
