So if the US hits one of China's man-made islands with a tactical nuke and then China responded by hitting a US aircraft carrier with a tactical nuclear tipped cruise missile, that's okay?

Absolutely not. Guam, Diego Garcia, and Hawaii don't have enough people, if PLA is gonna be that timid they mind as well just use cruise missiles. China's nukes cannot be wasted on reducing ships and planes to metal hulks.
If USS Ohio fires a nuke, you hit Ohio... (no I'm not talking about the ship). Irony indeed. Works with entire USN's sub fleet.
In all seriousness what makes you think the US elite cares about their "general population" after covid its clear they could care less.. the elites all have bunkers and are fully prepped
China DF41 should have painted onto them names of US cities... like The Dallas, the Vegas, the Orlando, the Seattle...
also nukes are not that expensive nor hard to build, its 1960s technology... this is why China must catch up quantitaively with stockpile parity to us and ussr

It appears the US proponents for tactical nukes is based on the faulty presumption that China will be parsimonious with its use of nukes; they base this on China's small number of warheads as well as its No First Use (NFU) doctrine.
NFU as a policy is about as meaningful as a chasity vow or abstinence until after wedding... when push comes to shove no nation lets policy get in the way of existential survival...
Its all the more reason China must get more quantity of nukes fast.

And also the reason why the us is forcing China to join START arms control treaty lately (don't really know whats the point though, obviously China won't agree and what can the us do to force China to sign to the agreement?)
Because the us know that the window of China nuclear vulnerability is closing fast

As of now since PLA at least officially don't have tactical nuclear weapons, its likely that they would just reply with a full sized nuke somewhere on US territory. If its just an island outpost that was nuked, then China probably would hit back on an American military base.
PLA behavior shows that they're at least somewhat ready to win a nuclear war. You don't invest many billions in missile defense and civilian/military shelters if you just want MAD. Building that shows willingness to consider survival after nuclear war, which smaller nuclear powers like France and England haven't done.
Given that China has tested more advanced tactical WMDs like neutron bombs, if PLA hasn't already an undeclared stockpile of tactical nukes, they could assemble a lot of them on short order. The only hurdle to that is political. In that case they could also use tactical nukes back.
