Likely shock induced rotor-pylon instability which can become worse due to ground effects. That likely lead to loss of control and subsequent disorientation leading to crash.
Helicopter and tilt-rotors despite efforts can experience this and many other kinds of highly nonlinear instabilities and it is not easy to even theoretically predict under what real world condition they can occur. Some can be predicted but others condition remain hidden.
These kind of things you can only learn during actual war.
That's why risk averse US pilots don't' shoot under such conditions. Since their trainers are likely US firms, I don't think they trained much in risk management during real world conditions.
Main problem is small nations don't go around testing 50 or 100kg warhead close to ground or in air that often during training. As a result they don't have comprehension of this kind of things.
Also nations with large armies carry out combined exercises, where individuals are exposed to large number of high explosive detonations.
Those who gone from a city state to US and back, would not know much about even what average nation's pilots know regarding the nature of various shock induced phenomena.
